Pregnant, anxiety and severly constipated?

I am 12 weeks and haven't pooped in almost a week. But when ii did last night it took me like 30 minutes and hurt so bad. I am so dizzy and have been throwing up to where it burns and hard to breathe. I have anxiety and that doesn't help the situation. I think something terrible is going to happen to me and my baby. What should ii do?
